     บทคัดย่อ Coronary heart disease (CHD) is a major cause of mortality and morbidity worldwide. Vascular endothelial dysfunction and oxidative stress play a key role in development of atherosclerosis and its complications. The present study was conducted to investigate endothelial dysfunction by using the forearm blood flow (FBF) technique. Body mass index, heart rate, blood pressure and coronary risk factors were evaluated. The Gensini score calculated from coronary angiogram of each patient was used to determine the severity of coronary atherosclerosis. A significant increase in serum triglycerides and decrease in serum HDL-cholesterol were found in patients with coronary artery stenosis (>50% in ≥ 1 vessel). Plasma malondialdehyde (MDA) was elevated in CHD patients, suggesting increased oxidative stress in these patients. A tendency to decrease in plasma nitric oxide metabolites in CHD patients indicates endogenous nitric oxide might decrease. This thereby modifies the coronary endothelial function in CHD patients. Moreover, the endothelial response to reactive hyperemia during the measurement of FBF was significantly decreased in CHD patients, reflecting the presence of endothelial dysfunction. Interestingly, the Gensini score was correlated with FBF and plasma MDA levels (r = -0.421, P<0.001 and r = 0.297, P = 0.014, respectively). Collectively, these results suggest that endothelial dysfunction and oxidative stress can be used as markers to identify the severity of coronary atherosclerosis in CHD patients.
