เนื่องจากในประเทศไทย ยังขาดการสำรวจการให้บริการทางกายภาพบำบัดในการดูแลเท้าผู้ป่วยเบาหวาน ขณะจำนวนผู้ป่วยเบาหวานเพิ่มขึ้นอย่างต่อเนื่อง ดังนั้นการศึกษานี้เป็นการวิจัยเชิงสำรวจโดยมีวัตถุประสงค์ เพื่อศึกษาลักษณะการให้บริการทางกายภาพบำบัดและบทบาทของนักกายภาพบำบัดในการดูแลเท้าผู้ป่วยเบาหวาน และ สำรวจความคิดเห็นในเรื่องปัจจัยใจที่เกี่ยวข้องต่อบทบาทการปฏิบัติงานได้อย่างราบรื่นของนักกายภาพบำบัดในการให้บริการดูแลเท้าผู้ป่วยเบาหวานของโรงพยาบาลในภาคตะวันออกเฉียงเหนือของประเทศไทย ทำการเก็บรวบรวมข้อมูลโดยการแจกแบบสอบถามไปโรงพยาบาลของรัฐในระดับทุติยภูมิและตติยภูมิทุกแห่งที่มี นักกายภาพบำบัดได้รับใบอนุญาตประกอบโรคศิลปะ ในภาคตะวันออกเฉียงเหนือของประเทศไทยจำนวน 307 แห่งผลการวิจัยพบว่า มีการตอบกลับแบบสอบถามจำนวน 221 ฉบับ คิดเป็นร้อยละ 72 มีโรงพยาบาลที่มีนักกายภาพบำบัดให้บริการดูแลเท้าผู้ป่วยเบาหวานเป็นหลักจำนวน 208 แห่ง คิดเป็นร้อยละ 94 ผู้ตอบแบบสอบถามเป็นเพศหญิงร้อยละ 85.1 มีอายุเฉลี่ย 30.8±5.7 ปี มีการศึกษาระดับปริญญาตรี คิดเป็นร้อยละ 91.4 อายุการทำงานด้านกายภาพบำบัดเฉลี่ย 6.9±5.4 ปีเกือบร้อยละ 50 เป็นผู้มีประสบการณ์ในการฝึกอบรมการดูแลเท้าผู้ป่วยเบาหวานส่วนใหญ่ (ร้อยละ 77.8) ผู้มีประสบการณ์ให้บริการในการดูแลเท้าผู้ป่วยเบาหวานในระยะ 1-5 ปี และมาจากโรงพยาบาลชุมชนที่มีขนาดเล็ก ถึง ใหญ่ (ร้อยละ 76.4) ทีมสหสาขาวิชาชีพมีนักกายภาพบำบัดที่ให้บริการดูแลเท้าผู้ป่วยเบาหวานเป็นหลัก คิดเป็นร้อยละ 94.1 โดยเฉลี่ยมีนักกายภาพบำบัด 2 คน ต่อโรงพยาบาล และระยะเวลาในการดูแลเท้าผู้ป่วยเบาหวานโดยเฉลี่ย 4 ปี วิชาชีพพยาบาล เป็นทีมสหสาขาวิชาชีพที่ร่วมให้บริการดูแลเท้าผู้ป่วยเบาหวานด้วยมากที่สุด (ร้อยละ 79.2) สำหรับบทบาทการให้บริการทางกายภาพบำบัดนั้น ส่วนใหญ่มีการให้บริการครอบคลุมทั้ง 4 มิติ คือ ส่งเสริม ป้องกัน รักษาและ ฟื้นฟูสมรรถภาพ อย่างไรก็ตามยังมีการให้บริการต่ำกว่าร้อยละ 20 ได้แก่ 1) ด้านการตรวจด้วยเครื่องมือ เช่น ankle brachial index และซ้อมเสียง และ2) ด้านการรักษาด้วยการขจัดหรือขูดหนังหนา การทำ off loading และ การรักษาแผลติดเชื้อ สำหรับความคิดเห็นในเรื่องปัจจัยใจที่เกี่ยวข้องต่อบทบาทการปฏิบัติงานได้อย่างราบรื่นของนักกายภาพบำบัดในการให้บริการดูแลเท้าผู้ป่วยเบาหวาน ส่วนใหญ่ (ร้อยละ 80) เห็นด้วยมากถึงมากที่สุดในทุกๆ ข้อ จากการศึกษาสรุปได้ว่า บทบาทของนักกายภาพบำบัดในการให้บริการดูแลเท้าผู้ป่วยเบาหวานมีความครอบคลุมทั้ง 4 มิติ รวมทั้งมีความเห็นว่า การให้บริการได้อย่างราบรื่นได้นั้น ต้องได้รับการสนับสนุนจากรัฐบาล
Abstract: There has not been a study in Thailand regarding Physical Therapy services for diabetic foot care although the number of diabetic patients has increased. This is a survey research aimed to explore the category of Physical Therapy services, roles of physical therapists, and their opinion on factors that may facilitate the role of physical therapists in caring of patients with diabetic foot in north-eastern hospitals in Thailand. Three hundred and seven questionnaires were distributed to secondary and tertiary care hospitals with physical therapists licensed in therapeutics in the North-east region of Thailand to request for information on their roles and category of services for diabetic patients with foot ulcers. Two hundred and twenty one questionnaires (72%) were responded. We found 208 hospitals had provided physical therapy services for these patients. Most of the survey respondents were female (85.1%) with an average age of 30.8±5 years and 91.4% holds a bachelor degree. Almost 50% had 6.9±5.4 years working in physical therapy. Most of them (77.8%) had 1-5 years of experience on diabetic foot care and worked in small to large sized primary hospitals (76.4%). There are 94.1% of hospitals with multidisciplinary teams with physical therapists mainly providing care to diabetic foot patients. By average, 2 physical therapists per hospital and 4 years provided this service. Nurse was the most common multidisciplinary team to work with. Roles of physical therapy service, most of physical therapy services covered 4 dimensions of care including promotion, prevention, treatment, and rehabilitation. However, less than 20% of the services were done, which are 1) for examination, using specific equipment such as an ankle-brachial index and a turning folk and 2) for treatment, providing specific diabetic foot care such as callus removing, off-loading orthotic and infected wound care. Most of the respondents (80%) conceded about all factors that may facilitate the role of physical therapists in providing diabetic foot care. In conclusion, the role of physical therapists in diabetic foot care has control over all 4 dimensions. In addition, physical therapists also agree on that the supports from the government facilitate the service of diabetic foot care.
