     Abstract Systemic sclerosis (SSc) is a connective tissue disease that commonly leads to pulmonary hypertension (PH). N-terminal pro-brain natriuretic peptide (NT-proBNP) levels provide clinicians with prognostic information during follow-ups in PH patients. Differences in NT-proBNP levels between patients with SSc-associated pulmonary arterial hypertension (SSc-associated PAH) and patients with SSc–related pulmonary hypertension associated with lung disease (SSc-related PH- LD) have never been studied previously This study examined presence of such differences in the two subgroups of SSc patients. Ten SSc-associated PAH and 12 SSc-related PH-LD patients aged 38 to 73 years with World Health Organization Functional Class II and III PH were enrolled to this study. All patients underwent right heart catheterization and spirometry. NT-proBNP levels were measured by Elecsys proBNP ELISA. Mean differences and 95% confidence intervals (CI) of forced vital capacity (%predicted), mean pulmonary arterial pressure (mm Hg), and pulmonary vascular resistance (Wood units) were 11 (2 to 21), 11 (7 to 15), and 3.4 (1.5 to 5.3), respectively. Median difference and 95% CI of NT-proBNP level (pg/ml) was 2035.5 (355 to 3341). All parameters were significantly higher in SSc-associated PAH patients than in SSc-related PH-LD patients. The present study suggests that NT-proBNP levels may be ideal for differentiating between patients with SSc-associated PAH and SSc-related PH-LD, as NT-proBNP levels were 16.1 times higher in the former group of patients.
