     บทคัดย่อ Introduction and Objective Positive expiratory pressure (PEP) breathing can reduced dynamic hyperinflation (DH) and improved exercise capacity in COPD patient [1]: but, almost design of PEP device was unsuitable for dynamic exercise. New PEP device was developed in our laboratory for reduce DH during dynamic exercise. The aim of this study was to fine out the optimum design of conical-PEP device. We hypostasized that conical length, outlet orifice diameter and flow rate affected positive pressure. The conical-PEP design has to generate positive pressure within therapeutic range (5 – 20 cm H2O)[2] at resting (0.2 to 0.4 L.sec-1) and exercised flow rate (0.5 to 1.0 L.sec-1) [1]. Methods Five, six and seven mm outlet orifice diameter with 0 mm (a simple PEP device) and 10 mm in length (conical-PEP devices) were assessed. The BIOPAC MP 36 system was set to explore flow rate and positive pressure in all PEP devices and a control circuit. This system compile of a 50 cm corrugate tube, a bacterial filter for distributing air steam from an air pump, an airflow transducer, a one-way vale T-piece, a pressure cuff transducer and PEP device. We tested positive pressure response of each PEP device for three times to the differences flow of 0.1, 0.2, 0.3, 0.4, 0.5, 0.6, 0.7, 0.8, 0.9, 1.0, 1.1, 1.2, 1.3, 1.4 and 1.5 L.sec-1 at 500 Hz of data sampling. Results Results show that the positive pressure exponentially rise up following the increase of flow in each PEP patterns, but the pressure not increase in control circuit. Five mm diameter of simple PEP device represented the highest pressure; while the conical-PEP device with 7 mm diameter shows the lowest pressure compared with other PEP device. At resting flow rate, the simple and conical-PEP with 5 mm diameter showed pressure response at 1.66 - 11.06 and 1.43 – 9.58 cm H2O, respectively. During dynamic exercise flow rate, the optimum designs of PEP devices were 6 and 7 mm diameter (4.17 – 25.12 and 3.52 – 15.57 cm H2O, respectively) for simple PEP while the conical-PEP were 6 and 7 mm diameter (4.24 – 18.20 and 2.22 – 11.75 cm H2O, respectively). Conclusion This study indicated that the optimum designs of PEP device for resting flow rate was 5 mm diameter of the simple PEP and the conical-PEP and 6 to 7 mm diameter for dynamic exercise flow rate. In addition, it is not know that the simple and conical PEP would affect in other populations such as elderly people which have a physiological changes in respiratory system leading to mild DH development during exercise [3].
