Aims.
The study aimed to compare clinical outcomes and safety among cancer patients who used cannabis and morphine in dimensions such as survival rate, hospital admission rate, number of hospital days, quality of life, and side effects.
Methods.
A retrospective cohort observational study was conducted based on medical records from January 1, 2020 – March 30, 2023. A prospective cohort study using the EQ-5D-5L and ESAS quality of life scales was followed. Subjects were 98 living patients with cancer in the palliative care period of 6 hospitals in northeast Thailand.
Results.
The results show that the median survival time of the cannabis group (CG) was 0.69 years (95%CI, 0.333 - 1.210), and the morphine group (MG) was 0.52 years (95%CI, 0.333 - 1.210), p-value of 0.168. The CG had fewer hospital admission rate than the MG, p>0.05. The CG had fewer hospital days than the MG, p<0.05. The quality of life (QOL) assessment by ESAS found that the CG had a better QOL than the MG, p<0.05. The EQ-5D-5L tool assessed that the QOL of the CG had a better score than the MG, p<0.05. The MG had higher liver enzyme, i.e., AST values (Median: 95%CI = 35.5: 42.43-118.96) and ALT values (Median: 95%CI = 88.50: 122.26 – 449.75) than those who received cannabis, p<0.05.
Conclusions.
Patients with cancer who used cannabis had better clinical outcomes without serious adverse effects. Medical cannabis should be encouraged for palliative cancer patients.
