Abstract ID 363 The Development of a Clinical Nursing Practice Guideline for Acute Stroke Patients with Dysphagia
Introduction: The number of acute stroke patients has been increasing over years. Dysphagia is a condition that may develop after stroke and lead to a serious complication, i.e. aspirated pneumonia. Proper nursing practice is needed to prevent such complication.
Objectives: This study aimed to use the evidence to develop a clinical nursing practice guideline (CNPG) for acute stroke patients with dysphagia.
Method: The development process of CNPG was adapted from the evidence-based practice model (Soukup, 2000). Based on PICO (population, intervention, comparator, and outcome) format, relevant research and empirical evidence were searched from several well-known databases. The level of evidence developed by the Joanna Briggs Institute (2014) was used to determine the strength of evidence. All evidences were analyzed and synthesized to generate recommendations for the guideline. The Appraisal of Guidelines for Research and Evaluation II criteria was used to evaluate the guideline’s quality. Content validity of the guideline was examined by five experts.
Results: Seventeen pieces of evidence were obtained. Based on an analysis and synthesis, the recommendations for developing CNPG comprised of four phases: 1) swallowing screening; 2) preparation before eating (oral cavity and neck exercise and swallowing technique); 3) nursing care for safe swallowing; and 4) assessment for aspiration and pneumonia. The CNPG was accepted for its quality and validation.
Conclusion: This CNPG should be implemented in nursing practice and be taken part of nursing care to avoid workload. In addition, it should be tested through outcome research and long term outcome measurement.
