Title of Article Vitamin C supplementation improves blood pressure and oxidative stress after acute exercise in patients with poorly controlled type 2 diabetes mellitus: a randomized, placebo- controlled, crossover study

     Abstract Abst rac t : This study aimed to asses s the effect of vitamin C on blood pres sur e (BP) , and subseque n t ly on oxidative stres s and nitric oxide releas e, following the lowintensi ty exercise in the patient s . This study included 24 patient s with type 2 diabet e s mellitus (T2D) (age, 53 ± 7 yr; hemoglobin A1c, 10.1% ± 0.9%) randomiz ed into two six-week daily arms based on consumpt ion of eithe r placebo or 1,000 mg of vitamin C. The crossove r trial occur r ed after a six-week washout . Before and after both the suppleme nt a t ion arms, all patient s performed cycling exercise at 33% of peak oxygen consumpt ion for 20 min. BP were measur e d before, immedia t ely, and 60 min after the exercise. Blood samples were drawn before and immedi at ely after the exercise to determine plasma ascorba t e , malondialdehyde, F 2-isopros t an e s , and nitric oxide concent r a t ions . Data showed significant lower BP in the vitamin C arm when compa r e d with the placebo arm (SBP P < 0.001 at every time point, DBP P < 0.001 except at immedi at ely after exercise, P < 0.05). Plasma ascorba t e concent r a t ion (P < 0.05 at every time point) and plasma nitric oxide (at resting P < 0.001, immedia t ely after exercise P < 0.05) were significantly incre a s e d in the vitamin C arm than in the placebo arm . Plasma malondialdehyde (P < 0.05 at every time point) and F 2-isopros t an e s (P < 0.05 at every time point) concent r a t ions were significant ly lower in the vitamin C arm than in the placebo 1 arm. In addition, data showed significantly lower SBP (P < 0.001 at every time point ), DBP (P < 0.001 except at immedi a t ely after exercise P < 0.05) , plasma malondialdehyde (P < 0.001 at every time point) , and F 2 -isopros t ane s (P < 0.05 at every time point ) at post- suppleme n t a t ion than at pre- suppleme nt a t ion. Besides, ther e were significant ly highe r plasma ascorba t e (P < 0.05 at every time point ) and nitric oxide (at res t P < 0.01, immediat ely after exercise P < 0.05) concent r a t ions at post- suppleme nt a t ion than at pre- suppleme nt a t ion. This is in cont r a s t to the placebo supplemen t a t ion arm which demons t r a t e d no statis tical differenc e in all outcome s throughout the experiment . This study sugge s t s that six-week vitamin C suppleme n t a t ion decre a s e d pre- exercise and post- exercise BPs, possibly due to improved oxidative stres s and nitric oxide releas e. Howeve r , exercise had no effect on any outcome measur e s .
