     Abstract High blood pressure is one of the risk factors for cardiovascular disease (CVD). High peripheral vascular resistance and increase in cardiac function are two fundamental causes. Mutation in MTHFR gene, leading to the alteration of MTHFR activity, contributes to the elevation of blood homocysteine (Hcy) levels. Hyperhomocysteinemia has been known to affect endothelial function. The aim of this study was to investigate the association between MTHFR gene polymorphism C677T (rs1801133) and the risk of high blood pressure in Thais. We recruited 655 Thai subjects, aged ≥60 years, who attended for annual health check-up at Health Center 15, Bangkok. MTHFR C677T was genotyped using PCR-RFLP. The MTHFR C677T genotype frequency was significantly different between groups with normal diastolic blood pressure (DBP<80mmHg) and elevated DBP (≥80mmHg). TT genotype and T allele were associated with the risk of having elevated DBP with respective adjusted ORs: 2.53 (95%CI: 1.01-6.35), p=0.048 and 1.51 (95%CI: 1.10-2.09), p=0.011. By separating age groups, however, the association existed only in age group of ≥65 years [adjusted ORs: 5.23 (95%CI: 1.69-16.21), p=0.004 and 1.89 (95%CI: 1.30-2.76), p=0.001, respectively]. There was no association of this polymorphism with systolic blood pressure (SBP). In conclusions, MTHFR C677T was associated with elevated DBP in Thai subjects especially whose age ≥65 years. T allele might increase the risk of having high blood pressure, in part, by the elevation of Hcy level.
