     Abstract Abstract Background: Alcohol use disorders are one of the worldwide problems and frequently cause many health behavioral, psychosocial, socioeconomic problems. The episodes of alcohol related problems in male are higher than woman in the same year’s group. Objective: To describe the prevalence of alcohol use disorders and determine the association between alcohol initiation and alcohol use disorders (AUD) among working age males at Pa-O self-administered zones, Myanmar. Methodology: This cross sectional analytical study was conducted in Pa-O Self-administered zones, Myanmar. The total 450 respondents of working age males were selected by using multistage sampling method. Data was collected by using a structured questionnaire interview and analyzed by simple and multiple logistic regressions. Result: The prevalence of alcohol use disorders among working age males of 450samples was 38.67 %( 95% CI: 34.15-43.18). The average age at the time of alcohol initiation was 19.63 ± 4.29 years old. After controlling the confounding factors, drinking alcohol at the first time with friends (adj. OR= 4.41, 95% CI=2.51-7.74; p-value <0.001), drinking alcohol at the first time by invitation of friends (adj. OR= 9.76, 95% CI =5.21-18.28; p-value=<0.001) were significantly associated with AUD. Other factors that were associated with AUD were underweight (BMI ≤18.5) (adj. OR=5.83, 95% CI=2.91-11.69, p-value=<0.001), lived nearer to alcohol outlets (<=1000m) (adj. OR=8.06, 95% CI=4.61-14.08, p-value=<0.001).
