     Abstract Background and aims: Low back pain (LBP) is one of four major musculoskeletal conditions, the most prevalent of musculoskeletal conditions, and cost more than other condition. Lumbar instability (LI) was considered as one of chronic LBP (CLBP) subgroup which could be classified into clinical lumbar instability (CLI) and non-clinical lumbar instability (NCLI). CLI could be classified into radiological lumbar instability (RLI) and non-radiological lumbar instability (NRLI). LI could affect in pain, muscle spasm, functional disability and quality of life which had been reported. However, the evidence on prevalence of lumbar instability have different methodology. The aimed of this study was to identify the prevalence of lumbar instability in patients with chronic low back pain. Methods: Fifty participants aged between 20 – 60 years, participated in this study. Data was collected by using interview for demographic data and flexion-extension radiograph for diagnosis instability. Results: This study found that the prevalence of CLI, RLI and NRLI were 92%, 14% and 78%, respectively. Age of participants with CLI range from 20-59, participants with RLI range from 20-50, and participants with NRLI range from 20-59. The previous symptom of CLI participant persisted at least 3 months to 120 months. Conclusions: The finding of age and LBP period in this study demonstrated that LI can be occurred in people who aged from 20 years onward. Therefore, the early carefulness, prevention, and treatment of LBP should be recognized.
