     Abstract Traditional Thai Massage (TTM) has been known as an alternative treatment for well-being relies on the balance of life energy via an invisible pathway. We hypothesized that TTM could improve health by improving immune status. However, the evidence of TTM in immunity improvement is still unclear. The aim of this study was to evaluate the effect of TTM on the changes of immune cell subpopulations and cytokine productions in the elderly who have impaired immune functions. The immunological parameters at baseline were compared with those of the short-term effect of 60 minutes after a single TTM and the long-term effect of having TTM every week for 5 weeks. The study design was a crossover randomized controlled trial with a 1-month washout period. Eight participants were randomly allocated into either treatment or placebo groups. The flow cytometry was used to identify immune cell subpopulations based on cell surface markers. IL-17 and IFN-γ productions in CD4 T cell subsets were also evaluated. The blood samples and PBMCs were stained with monoclonal antibodies (mAbs) conjugated with different fluorochromes and analyzed by FACS Canto II. The subpopulation analysis included CD4 T cell subsets, CD8 T cell subsets, NK cell subsets and NKT cell subsets. The preliminary data indicated significant decrease of NK cells after TTM for 60 mins (P < 0.05). Other parameters, especially, IL-17 and IFN-γ productions in CD4 T cell subsets are under investigations. Consequently, this study demonstrated the appropriate study design and analytical method using flow cytometry to evaluate the effect of TTM on immune status in the elderly.
