     Abstract Introduction: Cholangiocarcinoma (CCA) is a malignant tumor and slow progression of the bile duct, which makes it difficult for early diagnosis, and most patients are detected in advanced stages. Cytokine-induced apoptosis inhibitor 1 (CIAPIN1) is a newly identified cytokine-induced apoptosis inhibitor, which has prognostic value for human tumors. Our previous study, we found that CIAPIN1 expressed only in papillary CCA tumors; however, its expression in CCA serum remains unclear. This study aims to evaluate CIAPIN1 as a bio-marker in CCA serum. Methods: Studying the expression of CIAPIN1 in CCA patients, tissue samples were analyzed using western blot, immunohistochemistry, and Human Protein Atlas. SecretomeP by using NN-score >0.5, SignalP by using likelihood ratio, and European Molecular Biology Open Software Suite (EMBOSS) by using cut off 3.5 as predicting signal peptide cleavage sites and the Plasma Proteome Database (PPD) were used to predict secretory protein nature of CIAPIN1 in CCA sera. To validate CIAPIN1 as a bio-marker for CCA, serum CIAPIN1 levels of 48, 22, and 22 samples collected from CCA, benign biliary diseases (BBD), and healthy control groups, respectively, were measured using dot blot analysis. Results: IHC and western blot revealed CIAPIN1 was expressed in CCA tissues. Bioinformatic tools predicted CIAPIN1 sequence has one signal peptide cleavage sites and could be present in the serum as well. Dot blot results showed serum CIAPIN1 level in CCA and BBD group was significantly higher than that in the healthy control group. Conclusions: The significance expression of CIAPIN1 in CCA tissue and serum was demonstrated. Dot blot results showed serum CIAPIN1 level in CCA and BBD group was significantly higher than that in the healthy control group. Therefore, CIAPIN1 should be further investigated in the serum with clinical factors of CCA patients as a diagnosis/prognosis bio-marker.
